Open Enrollment for January

You do not need to take any action. You will be automatically enrolled in the same health insurance plan with the same carrier and coverage.

If you would like to do any of the following, you must complete enrollment form(s) and return them to Human Resources no later than 4:00 p.m. on Monday, Nov. 28.

• Change plans (advantage high, advantage value or HSA compatible plan)
• Change insurance carriers (Preferred One, Blue Cross Blue Shield, HealthPartners)
• Change your coverage significantly, like by adding or removing dependents (Decline, Single, Single +1 or Family)
• Enroll in the HSA (Health Savings Account) if you elected the HSA compatible plan. Note: An HSA account is similar to a savings account for qualified health plan expenses and is provided through vendor Further.

Forms must be received by Human Resources no later than 4:00 p.m. on Monday, Nov. 28 for changes to be processed. Human Resources recommends completing forms online, then emailing them to HR@district279.org. You may also fax directly to the Human Resources confidential fax number, 763-391-7224. Please do not send forms via inter office mail or send multiple copies.
